CHS Planning Guidelines for Local Public Health
The Community Health Services in Minnesota has identified the following as Key Indicators and Performance Measures to gauge how well we are doing at the local level.

Assure an Adequate Local Public Health Infrastructure
- Contains components that are required by law for Community Health Boards and prevents the deterioration of the Public Health infrastructure.

Promote Healthy Community and Health Behavior
- Applied to behaviors including physical activity, overweight and obesity, tobacco use, alcohol and substance abuse, mental health, unintended and intended pregnancy issues, child and adolescent growth and development, disability and decreased independence, and injury and violence.

Prevent the Spread of Infectious Disease
- Focus on infectious diseases that are spread person to person. Contains activities of detecting acute and communicable diseases, developing and implementing prevention of disease transmission, and implementing control measure during outbreaks.

Protect Against Environmental Health Hazards
- Addresses aspects of the environment that pose risks to human health, but does not include injuries. Include activities that identify and mitigate environmental risks at the source and includes asbestos, lead, radon, radioactivity and radiation, ground/surface water and wells, food borne and waterborne diseases, air contamination, Public Health nuisances, and occupational diseases.

Prepare for and Respond to Disasters, and Assist Communities in Recovery
- Activities that prepare Public Health to respond to disasters and assist communities in responding to and recovering from disasters.

Assure the Quality and Accessibility of Health Services
- Assesses health care capacity and access to health care, identifies and fills health care gaps and links people to needed services for all citizens with their population.
